Getting your information from the Health Provider Portal
The Health Provider Portal contains patients' health records including:
- discharge summaries
- medical imaging reports
- medication list
- outpatient letters
- operation reports
- pathology reports
- test results.

You may also be able to find the information you need on My Health Record.
Getting your information from us
Using the Health Provider Portal is the preferred way to access your patient's health records, however you can also contact our Health Information Unit at SWHHS_Health_Information@health.qd.gov.au.
